For your company’s use: This template helps your managers and People team evaluate the pilot. Your organization controls collection and owns its responses. You may adapt or skip questions. Sharing survey responses or results with BayWel is optional and is not a condition of receiving the pilot materials or rollout credit.

Optional BayWel feedback
If you choose to share feedback with BayWel, one question is enough: “How likely are you to recommend Mindful Work to another manager?” (0–10; 0 not at all likely, 10 extremely likely). No additional BayWel feedback is required.
Company measurement framework
- Activation: managers reporting 1+ uses ÷ responding managers; also show invited-manager denominator.
- Used at least twice: managers reporting 2+ uses ÷ responding managers.
- Frequency: show the 0, 1, 2, 3, 4, 5+ distribution. A mean coded with 5+ as 5 is a lower-bound estimate, not an exact mean.
- Ease and usefulness: arithmetic mean of valid 1–5 answers. Report the answer count for each.
- Continuation: Yes ÷ valid Yes/Maybe/No answers; report Maybe separately.
- Recommendation NPS: percentage rating 9–10 minus percentage rating 0–6; 7–8 are passive. Report sample size. Keep raw average recommendation separate.
Report response rate, missing data and limitations. Nonrespondents may differ from respondents. Obtain permission before using anonymous comments externally.
